THE WHISTLEBLOWER PROGRAM WANTS YOUR INPUT

The Office of the Controller's Whistleblower Program is committed to providing quality service to City officers, employees, and members of the public. As part of our goal to improve services to complainants, the Whistleblower Program will launch a complainant survey in fiscal year 2014-15.

WHO CAN COMPLETE THE COMPLAINANT SURVEY?

Anyone that filed a complaint with the Whistleblower Program on October 1, 2014, or later.

WHEN CAN I COMPLETE THE SURVEY?

The survey will be available once your complaint is closed.

HOW DO I COMPLETE A SURVEY?

Go to the Whistleblower Program's Status Check page and enter your complaint tracking number. Please note that tracking numbers are case sensitive.

If the complaint status is "Closed", complainants will see this link:

Clicking this link will take complainants to the survey. A complainant may submit only one response per complaint investigated.

WHAT WILL THE WHISTLEBLOWER PROGRAM DO WITH THE DATA?

The results gathered from this survey will be reviewed by the Whistleblower Program to improve services. Results gathered from survey responses may be published in future Whistleblower Program reports with any identifying information removed.
